Good Faith Estimate Notice

As part of our commitment to transparency and client-centered care, we provide you with a Good Faith Estimate (GFE) of the anticipated costs associated with your treatment. This estimate is provided in accordance with the No Surprises Act (NSA) enacted by the U.S. Department of Health and Human Services.

Factors Affecting Costs: The total cost of services can vary based on several factors, including the type and duration of sessions, the complexity of your presenting concerns, any additional assessments or evaluations required, and the recommended frequency of appointments.

Insurance Coverage: We encourage you to contact your insurance provider to verify your coverage for mental health services. Coverage levels and reimbursement rates may vary. We are committed to assisting you in navigating the insurance process and maximizing your benefits.

Good Faith Estimate Disclosure: Upon request, we will provide you with a personalized Good Faith Estimate outlining the anticipated costs of your treatment plan. This estimate will include details such as session fees, assessment charges, and any other applicable expenses based on your specific treatment needs. If you receive a bill that is at least $400 more for any provider or facility than your Good Faith Estimate from that provider or facility, you can dispute the bill.
